Please have all medical history for your pet. You can have the previous clinic email us at foresthillsbethospital@gmail.com. We ask that you have your pet's history sent prior to your appointment or bring information to your appointment. Please arrive 10-15 minutes prior to your appointment so we are able to review your pet's previous history.
